Mobile-PASS: I had to reset my phone to factory settings - have I lost my license?

Yes. 

For security reasons, if you restore your phone to factory default settings, any and all of your Mobile-PASS licenses in the ATRIUM BT app will be permanently deleted

You will need to contact your system administrator so that they can send you a new Mobile-PASS license.
